to BioSITESAFE, each contractor/vendor is responsible for the costs
associated with required personnel screening, compliance
orientations/testing and credentialing.
BioSITESAFE subscribing biotech companies have negotiated the prices to
the lowest possible denominator, to minimize the impact on their
contractor/vendors.
This "Pay-to-Play" model enables BioSITESAFE subscribers to save
millions of dollars in screening and credentialing costs, without
sacrificing security.

- How do I get started?
You will need to become a BioSITESAFE subscriber by completing a
Customer Profile and Subscriber Agreement. Many BioSITESAFE subscribers
my require you to complete additional registration forms which are
available through this site.

- Why does BioSITESAFE require that

This information allows us to verify your business and protect any
companies that might try and impersonate you to gain access to
Biotech/Pharmaceutical facilities.

We only require a bank name, phone number and contact to verify
standing.

- Why do I have to get an on-site
certification?
Due to recent events involving information breaches, this protects you
as the client from any companies that attempt to steal your identity to
gain access
to Biotech and Pharmaceutical facilities.

- What should I do if I don't have
my vendor ID?
Don't worry. Simply submit the customer profile without it, but
remember
to indicate the Biotech/Pharmaceutical company or contractor/vendor
that you are associated with.

- How am I supposed to get pictures
of my employees to BioSITESAFE?
There are several methods to choose from:
- If you have a digital
camera simply take the picture and upload into the BioSITESAFE system
when prompted (remember to keep your camera on a low resolution so you
don't go over the 500kb limit)
- If you do not have a
digital camera, we can refer you to a site to purchase for around
$80. If you do not wish to use a digital camera, then you can
have employees bring in a photo of themselves (must be a clear face
shot, no hats, sunglasses etc.) and you can scan it then upload into
BioSITESAFE when prompted.
- If you do not have a
scanner, any local Wal-Mart, Walgreens, or any photo center can provide
this to you. You can upload several hundred photos onto a disk for just
a few dollars

- How much does this service cost?
There is a one time Contractor/Vendor set-up fee of $99.00 to access
BioSITESAFE functions. Additionally, Contractor/Vendors are responsible
for all costs associated with required personnel screenings, compliance
testing and security credentialing of their employees.
Applicants/Employees that successfully complete the program and are
deemed "eligible" receive
security credentialing (an access ID badge)
at a charge of $60/per person.
If an applicant/employee is deemed "ineligible"
to receive an access ID
badge, then the charge is $40

BioSITESAFE has already negotiated the prices to the lowest possible

- Why is there an extra $55.00
surcharge associated with some New York addresses?
In July of 2003, the state of NY changed their policy on reporting
felony and/or misdemeanor record information. For certain counties,
there is no logner a county seat available that reports felony or
misdemeanor records. All information is forwarded to the state level.
A NY statewide search is $65.00, however, all BioSITESAFE clients are
given a $10.00 discount to automatically order the NY statewide if
applicable. As a part of the BioSITESAFE program, the NY statewide is
required if any of the addresses yielded per employee turn up in the
specified restricted counties. Each BioSITESAFE client is notified of
the NY surcharge during the account setup process.

- What does this service do?
This program facilitates testing, background checks and badges required
by Biotech and Pharmaceutical companies that subscribe to BioSITEASAFE.

- Who is required to participate?
Each Biotech or Pharmaceutical company that subscribes to BioSITESAFE
requires its employees or contractor/vendor personnel to submit to
background checks and complete other compliance requirements to gain
access to key facilities or worksites.
Any applicant/employee who refuses consent for the personnel screening
or that does not complete compliance requirements are ususally denied
access to designated worksites.
Many BioSITESAFE subscribing companies maintain their own criteria for
granting or denying access to applicant/employees.

Laboratories
Abbott will require:
- Contractors to register
their company with the Program at www.biositesafe.com
- Contractors must complete
a safety questionnaire and provide safety documents in order to
demonstrate adherence to government and industry safety standards and
regulations.
- Contractors must secure
background consent/waivers and Abbott Non-Employee Questionnaires
("NEW") from each of their employees that will be assigned to work at
Abbott location. Any contractor's employee who refuses to provide
consent to a background check or complete the NEW will not be allowed
on site.
- Contractors will be
required to provide electronic photos of each employee for submittal to
the Program. The photo will be used to create an Abbott
identification/access badge. The employee will then complete an
on-line site safety orientation via the website.
- After the employee
successfully completes the orientation, an identity and background
investigation will be initiated.
- Upon successful
completion of these steps, a photo identification badges will be sent
to the Abbott site's security office.

- How are Applicants/Employees
denied or approved?
Each Biotech or Pharmaceutical company that subscribes to BioSITESAFE
has its own criteria for granting or denying access to
applicant/employees.

be denied access to an Abbott facility if they have a felony conviction
within the last 7 years, or they
can be denied if they have misdemeanor crimes of concern

- What if I fail the tests required
by my employer that are on BioSITESAFE?
Each Biotech or Pharmaceutical company that subscribes to BioSITESAFE
has its own criteria. Please select the company that applies to
you.

- Abbott requires all
Contractor/Vendor personnel to complete their orientation and pass the
corresponding test with a score of 80% or better in order to be deemed
eligible to access designated worksites and facilities.

- How long does it take to get my
ID badge?
Usually 2-3 businesss days depending on when you complete the
compliance orientations/tests and when your employer has uploaded all
of the required information. In most cases, the badges are issued
to your employer for final distribution.
Some Biotech or Pharmaceutical companies that subscribe to BioSITESAFE
may have their own procedure for obtaining the ID Badges for personnel.
